چکیده

THE investigation of nervous diseases caused by viruses requires close co-operation between clinical and experimental workers. The present paper is the summary of such co-operative work performed by a neuro-histologist and a team of workers in virus diseases. Its chief purpose is to throw some light upon the etiology and pathogenesis ofacute disseminated encephalomyelitis and multiple sclerosis, because up to now many unsolved problems remain, not only in the field of etiology but also in that of their morphology and pathogenesis. Some authors are still inclined to doubt the infectious origin of multiple sclerosis, and offer a number of theories attempting to explain the pathogenesis of this disease, such as Putnam's obstruction theory, and toxic, allergic, lipolytic, and other theories. We shall not dwell on the non-infectious hypothesis of multiple sclerosis, because the authors advocating this theory take the effect of the pathological process for its cause. These hypotheses are based on changes occurring in individual morphological elements (glia) or on individual normal or abnormal responses of the organism, and so fail to yield an outline of the whole pathological process; they thus cannot give a true idea of pathogenesis. The result of virus and post-mortem investigation of many patients suffering from acute disseminated encephalomyelitis and multiple sclerosis was that two strains of filtrable viruses were isolated from two cases of acute disseminated encephalomyelitis. These strains appeared to be identical, but differed from other known strains of neurotropic viruses encountered in man.

Tracing immature myelin in acute disseminated encephalomyelitis.

Inherent abnormalities of myelin have been suggested in the pathogenesis of multiple sclerosis (MS). We investigated myelin in acute disseminated encephalomyelitis (ADEM) patients by magnetic resonance spectroscopy (MRS) and cerebrospinal fluid (CSF) analysis for citrulline, a marker of immature myelin. A citrulline peak was observed in the normal appearing white matter of 7/15 patients and of ...

Multiple sclerosis vs acute disseminated encephalomyelitis in childhood.

The initial presenting clinical and laboratory findings of either acute disseminated encephalomyelitis or the first attack of multiple sclerosis in the pediatric population were compared and contrasted. A retrospective review of the medical records was conducted of all children younger than 17 years who presented with either the diagnosis of acute disseminated encephalomyelitis or multiple scle...

Detection of coronavirus in the central nervous system of a child with acute disseminated encephalomyelitis.

We present a case in which human coronavirus was detected in the cerebrospinal fluid of a child presumed to have acute disseminated encephalomyelitis. In murine models, coronavirus has been found to cause a chronic demyelinating condition that resembles multiple sclerosis. Additionally, there is in vitro evidence of human coronavirus's ability to infect neural cells. This case report provides a...

Perivenous demyelination: association with clinically defined acute disseminated encephalomyelitis and comparison with pathologically confirmed multiple sclerosis.

Distinction between acute disseminated encephalomyelitis and acute multiple sclerosis is often clinically difficult. Perivenous demyelination is the pathological hallmark of acute disseminated encephalomyelitis, whereas confluent demyelination is the hallmark of acute multiple sclerosis. We investigated whether perivenous demyelination versus confluent demyelination distinguishes acute dissemin...

Acute disseminated encephalomyelitis

Acute disseminated encephalomyelitis (ADEM) is an acute demyelinating disorder of the central nervous system, and is characterised by multifocal white matter involvement. Diffuse neurological signs along with multifocal lesions in brain and spinal cord characterise the disease. Possibly, a T cell mediated autoimmune response to myelin basic protein, triggered by an infection or vaccination, und...
